I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. The current tuition is stated on this form and is subject to change with notification. I understand that my recurring payment is non-refundable if I do not process the cancellation form online no later than 30-days before the next recurring payment.
ATTENDANCE AND MAKE-UPS: Tuition is based on four (4) weeks of class programming. This includes one (1) structured class and one (1) Pratice & Play per week. Members must inform the gym of an absence a minimum of 2 hours in advance to be eligible for make-up. Each make-up must be completed while enrolled in classes. Make-ups expire on the last day of your class or 12/31 whichever comes first. Any unused makeups cannot be used during a FREEZE session. There may be short periods when My Gym is closed or classes are not running. If this occurs and a class is missed, the enrolled student is entitled to make-up.
This agreement constitutes a continuous payment plan and will remain in effect until cancellation or freeze request is completed by the parent on file no later than 30-days before the next payment. Your child will automatically remain enrolled at My Gym until an official Cancellation Request Form* (CRF) or Freeze Request Form (FRF) has been submitted ONLINE* It is agreed that you will submit a CRF or FRF no later than 30-days before your next payment. If your CRF or FRF is not submitted within the specified time (no later than 30-days before your next payment), you are responsible for one final payment on the next scheduled date.
Members agree to remain enrolled until a cancellation form is submitted no later than 30-days before the next recurring payment. A Fee of $10 will be charged for all returned or declined payments. Any and all payments to My Gym are non-refundable and non-transferable under any circumstances.
