I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. I understand this constitutes a continuous payment plan and will remain in effect until terminated by the member. The current tuition is stated on this form and is subject to change with advance notification. You will automatically remain enrolled at My Gym until you cancel your enrollment from your My Gym account and receive an email confirmation from the gym stating your cancellation request has been processed. My Gym requires a 30-day notice to cancel. It is agreed that, upon cancellation, you may continue to attend the gym until your final day of membership.
By providing a card on file with the My Gym, you are granting permission for that card to be charged for approved My Gym-related purchases. These charges may include, but are not limited to, socks, merchandise, special events, activities, services, and other child-related expenses.
Teachers and authorized school staff are permitted to charge the card on file when these items or services are provided to your child. Charges will only be made for My Gym-approved purposes that support student participation and involvement.
Members must inform the gym of an absence in advance. There may be short periods when My Gym is closed and/or classes are not running. There are no refunds or credits for these closures. To the best of our ability, we will inform members in advance of these closures so that members may attend additional programs before or after the closure.

Cancellation instruction are the website and can be provided to you upon request via text. I understand that is is my responseabity to process the cancellation online.
Failure to attend, participate, process a cancellation or utilize services does not constitute cancellation or relieve payment responsibility.
Membership and tuition are nonrefundable and nontransferable. I have give a copy of the policies of My Gym via text message and I understand the policies posted public on the website and in the facility for my review.
