I hereby authorize My Gym to charge my credit/debit card every four (4) weeks (28 days).I understand this constitutes a continuous payment plan and will remain in effect until terminated by the member. I understand that if a payment is processed before making a cancelation request, the payment is non-refundable under any circumstances. Membership will be canceled at the requested time, and my child/children will be allowed to participate in all of the activities included in my membership for the time (4 weeks) paid. The current tuition is stated on this form and is subject to change with advance notification.
Members must inform the gym of an absence in advance. There may be short periods when My Gym is closed and/or classes are not running. Payments for all memberships will be advanced when the Gym is closed for 4 or more consecutive days to provide all members with “make up days” for the extended closure. There are no refunds or credits for these closures. To the best of our ability, we will inform members in advance of these closures so that members may attend additional programs before or after the closure.
This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. You will automatically remain enrolled at My Gym until you cancel your enrollment from your My Gym account and receive an email confirmation from the gym stating your cancellation request has been processed. My Gym requires a 30-day notice to cancel. It is agreed that, upon cancellation, you may continue to attend the gym until your final day of membership.
My Gym membership is a continuous payment every 4 weeks, whether a child attends classes or not.The parent/guardian is responsible for bringing their child/children to their classes and benefiting from the unlimited access, as well as terminating the membership if unable to participate in classes or for any other reason they may need to cancel the tuition. If your child/children are not attending classes, the system does not cancel the automatic payments. The parent or guardian must request an official cancelation to stop payments and cancel the continuous payment plan.
Membership and tuition are NONREFUNDABLE and NONTRANSFERABLE once payments have been processed.
