I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. I understand this constitutes a continuous payment plan and will remain in effect until terminated by the member. The current tuition is stated on this form and is subject to change with advance notification.
Members must inform the gym of an absence in advance. There may be short periods when My Gym is closed and/or classes are not running. There are no refunds or credits for these closures. To the best of our ability, we will inform members in advance of these closures so that members may attend additional programs before or after the closure.
This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. Your child will automatically remain enrolled at My Gym until an official Cancellation Request Form (CRF) has been completed and submitted with at least 28 days’ notice. It is agreed that, when a CRF has been received, the member will be responsible for one final payment on the next scheduled date. This payment may be the full tuition amount or prorated based on the number of classes left in the billing cycle.
Membership and tuition are nonrefundable and nontransferable.
