I hereby authorize My Gym to charge my credit/debit card every four (4) weeks. The current tuition is stated on this form and is subject to change with notification.
Tuition is based on 4 weeks of class programming. Members must inform the gym of an absence a minimum of 2 hours in advance to be eligible for a make-up. Make-up credits can only be used for enrolled children and siblings. There may be short periods when My Gym is closed or classes are not running. If this occurs and a class is missed, the member is entitled to a make-up. Unused makeup credits will expire after 3 months. Makeup credits can only be used when you are actively enrolled. They cannont be used when your account is frozen, and upon cancellation, they must be scheduled before your last class. If you schedule a make up class and do not cancel the class via your account or let us know you will not be attending, you will lose that make up credit. It will no longer be added back to your account, unless cancelled before the class.
This agreement constitutes a continuous payment plan and will remain in effect until terminated by the member. You will automatically remain enrolled at My Gym until you cancel your enrollment from your My Gym account and receive an email confirmation from the gym stating your cancellation request has been processed. My Gym requires a 30-day notice to cancel. It is agreed that, upon cancellation, you may continue to attend the gym until your final day of membership.
A Fee of $15 will be charged for all returned payments. Any and all payments to My Gym are non-refundable and non-transferable under any circumstances.
